After upgrade from 4.1.12 to 4.2.5 not appending "_fixed" to end of files

After I upgraded from 4.1.12 (I think) to 4.2.5 it appears to have stopped appending "_fixed" to the end of files. Settings still shows it should be set for that.

OK, we checked that but it worked as expected. As you can see, "_fixed" was appended to the file name.

How can I troubleshoot what might have caused mine to stop after upgrading?

Edit: I think I figured something out. It only appends _fixed if the file extension is the same.  So if I drop a file *.mpg (that Channels DVR uses) it saves to *.ts without appending. If I drop a *.ts file then it appends the _fixed add on. It used to append _fixed no matter what.
